Nature of action: This is a court proceeding requesting that the court enter an order declaring that the name of the above-named individual be changed from Blake Morgan Pyle to Blake Morgan Nielson. The hearing in this Petition has been scheduled to take place on February 10, 2014 at 1:30 p.m. in Courtroom No. 2 of the Lebanon County Court of Common Pleas, located at 400 South Eighth Street, Lebanon PA 17042.
